These tags are sent by USPS Priority Mail; a signature is required for delivery.

Suggestions for filling out form:
1. All information is optional.
2. Only put the most important medical information on the tag. As we are not medical professionals, it is recommended to consult with your doctor as to what should go on the tag if you are not sure. Additional info can be written on the included wallet card.
Do not include medication dosages. If dosage should change it will make the tag obsolete.
3. If you want your address on tag, use your street address; not a PO box.
4. Emergency phone number for the tag should not be your home phone number unless someone else lives there with you. Nor should it be your personal cell phone number.
5. For bracelets, it is recommended to add at least 1/2 inch to the wrist measurement.

Style "B" Small Oval Bracelet
All bracelets come assembled with clasp and chain.

Tag description:

All tags are available in Solid Surgical Stainless Steel, Solid Sterling Silver, or 14k Gold-filled. They are available with or without the medical symbol embedded in the tag.
The Style B is our smallest bracelet tag; it is preferred by women and children. At the widest points the tag measures 1 1/4 inch width, 1/2 inch height.
All bracelets come with link-chain and a very secure "Sister hook" clasp.

Engraving information: tag front has person's name and an emergency phone number, while tag back has medical conditions.
